About Community Care | Sheboygan County

Community Care | Sheboygan County is a nonprofit organization with over 40 years of experience providing individuals with long-term care needs the opportunity to live as independently as possible. Our Medicare/Medicaid long-term care programs offer a range of supportive services that enable adults with disabilities and older adults to live safely, confidently and with dignity. With more than 10,000 Wisconsinites entrusting us with quality support to reach their potential in health, happiness and independence, we serve 15 counties throughout the state. As a Wisconsin-based organization, we have local offices in each of the counties we serve for added convenience.
