Community Care | Outagamie County (Family Care Office) is a nonprofit organization with over 40 years of experience providing quality support for older adults, people with disabilities, and other Wisconsinites in need of long-term care. Our Medicare/Medicaid long-term care programs help individuals maintain independence and live with dignity. With 15 local offices throughout Wisconsin, we have helped more than 10,000 people live confidently, safely, and as independently as possible. We strive to provide the best possible support and services, so our clients can reach their full potential in health, happiness, and independence.
